THE WRAPAROUND PROGRAM PROVIDES A COMPREHENSIVE PROGRAM TO REDUCE THE RISK OF PLACEMENT BREAKDOWN FOR YOUTH IN CARE, AND DEVELOP THE CAPACITY OF THEIR FAMILY-BASED CAREGIVERS TO MANAGE THEIR CARE ON AN ONGOING BASIS. THE PROGRAM PROVIDES DIRECT INTERVENTION AND SUPPORT SERVICES TO REFERRED CHILDREN IN CARE AND/OR THEIR FOSTER FAMILIES AND CASE MANAGEMENT SUPPORT TO OTHER SERVICE PROVIDERS INVOLVED.
THE FOCUS IS TO HELP THE CHILD OR YOUTH AND FAMILY INCREASE THEIR:
Referrals for the Program are accepted from the Guardianship social worker, as well as the Resources social worker, and are screened for suitability, prior to admission.
After receiving a referral, we contact the youth and caregiver to discuss the program, and answer any questions. An initial meeting is then set up, and at that time, the plan for service begins.
Duration and frequency of service depends on the individual needs and goals of each family.
