Program Overview

The Youth Outreach program serves youth aged 12-18 who are at-risk of becoming disengaged from their families, community, positive peers, pro-social activities, and school in Trail, Castlegar, Salmo, and the Slocan Valley. The Program collaborates with MCFD and community service providers using an Integrated Case Management model to address risk factors and to increase safety, connection and belonging for youth within their families and communities. Youth Outreach may focus on referred youth and/or their parent(s)/caregiver(s), depending on assessed needs. The Youth Outreach Program provides support to youth and their families in the following areas:
Youth Outreach Reconnect Services: Support to youth who are at risk of becoming disengaged from their families, community, positive peers, pro-social activities, and school.
Independent Living Skills Acquisition: Support to enhance daily living skills of youth working towards independence, or to better support youth in the home, community, at school, and with peers. The repatriation and/or continued connection to family is a service priority. Services are provided on a flexible outreach basis, in the homes and communities of the referred youth.
Youth Outreach also offers short-term and intensive Caregiver/Parent-Teen Mediation. This part of the program is focused on supporting referred youth and their caregiver/parents that are experiencing significant conflict and require extra supports. The primary focus of this service is to provide the opportunity for the entire family to develop healthy coping strategies and communication, while encouraging positive family relationships, including repatriation and connection.
Referrals for the program are accepted from MCFD, schools and community agencies. Self-referrals from youth and their families are also welcome.
