Support to Family Care Network provides support, facilitation and training services for family care homes.

Goals:
Develop the capacity of caregivers to provide for the unique needs of the children in their homes, to build connections with other family care home providers and help navigate the system in which they work.

Permanency Support provides individual and family counselling, behaviour support, case management, service coordination, and follow-up support.

Goals:
Strengthen caregiver capacity as children and youth transition from care to an adoptive or extended family home.

Supervised Access Services supports families to maintain contact in a healthy and safe environment.

Reduce the risk of harm to children during mandated supervised visits and to help caregivers better understand the intentions behind their children’s behaviours.

Family Advancement Services provides short term, intensive, outcome-focused support.

Goals:
Increase the harmony in the home by increasing parenting capacity and the parent-child relationship.

Youth Support Services provides individualized transitional support services.

Goals:
Rebuild the connection between referred youth and their families and/or connect them to relevant community supports.

Youth Outreach Services provides individualized outreach support.

Improve social relationships and connection to healthy social community resources and activities

Independent Living Skills Acquisition provides a flexible outreach service.

ISSP provides an intensified level of community supervision and support to monitor compliance with youth probation order conditions.

Goals:
Reduce or eliminate future criminal activity by reducing risk and increasing protective factors in the lives of referred youth.

• Charged with a serious criminal offence
• On a youth bail or probation order

PSI provides individualized outreach support services for adults.

Goals:
Increase quality of life in the areas of shared living, community inclusion, employment skills and life skills development.

Eligibility

• Adults with significant limitations in adaptive functioning AND
• A Fetal Alcohol Spectrum Disorder (FASD) diagnosis 
  
  and/or an autism spectrum disorder diagnosis 
• OR an Intellectual Disability diagnosis
